Depression and other mood disorders are very common conditions experienced by a large percent of our population. While many avoid seeking treatment due to the belief that they should be able to manage on their own or due to concerns about stigma, depression and other mood disorders are like any other medical conditions and can be effectively treated when appropriate treatment is sought.
At Shugarman Psychiatric & Counseling, our board-certified psychiatrists and licensed marriage and family therapist provide comprehensive care for depression and other mood disorders for persons living in Alexandria, Virginia and the Greater Washington, D.C. area, using a combination of psychotherapy and psychiatric medication, with the treatment approach individually tailored to the needs of our patients.
In order to afford sufficient time to arrive at accurate diagnoses and to better understand you as a person, evaluations with our psychiatrists are two hours in length, and assessments with our therapist are one to two hours in length. For appropriate patients, follow-up care may be provided through telepsychiatry (telehealth) video visits following an initial in-person evaluation.
What Depression and Mood Disorders Can Look Like:
Depression and other mood disorders can affect mood, energy level, motivation, productivity, concentration, sleep, self-concept, relationships, and overall quality of life. While some individuals with depression experience persistent sadness or loss of interest in activities, others may present with irritability, emotional numbness, chronic exhaustion, difficulty concentrating, or a sense of feeling overwhelmed by everyday responsibilities. Symptoms may develop gradually over time or emerge more abruptly during periods of increased stress, loss, burnout, interpersonal difficulties, or major life transitions.
Mood disorders exist on a spectrum and can vary considerably from person to person. Some individuals experience isolated depressive episodes, whereas others experience chronic low mood, recurrent depression, or more significant mood instability.
For individuals with bipolar disorder, mood symptoms may fluctuate between periods of depression and periods of elevated mood, increased energy, impulsivity, decreased need for sleep, racing thoughts, or irritability. Some individuals may also experience psychotic symptoms such as hallucinations or delusions (strongly held false beliefs that are not reality-based). Bipolar disorder is sometimes initially mistaken for depression alone, particularly when individuals seek treatment primarily during depressive episodes. Accurate diagnosis is therefore important, as treatment approaches for bipolar-spectrum conditions often differ from those used for depression alone.
Depression and Mood Disorders Frequently Occur Alongside Other Mental Health Conditions:
Symptoms of depression and other mood disorders can overlap with other psychiatric and psychological conditions, including anxiety disorders, chronic stress, trauma-related conditions, ADHD, sleep disturbances, and substance use disorders. In some cases, symptoms that initially appear to reflect depression or bipolar disorder may instead be related to another underlying psychiatric or medical condition, though it is also common for individuals with mood disorders to experience multiple mental health conditions simultaneously.
Comprehensive psychiatric evaluation is important in order to accurately distinguish mood disorders from other conditions that may produce similar symptoms, while also identifying situations in which multiple conditions may coexist.

We obtain an in-depth history, carefully assess mood symptoms over time, and continually reevaluate to optimize diagnostic accuracy and treatment planning.
Treatment for depression and other mood disorders may involve psychotherapy, use of psychiatric medication, lifestyle modification, or a combination of approaches, depending upon the nature and severity of your symptoms and individual preferences. Some individuals with depression benefit primarily from psychotherapy and supportive interventions, whereas others may benefit from psychiatric medication alone or in combination with psychotherapy. For individuals with bipolar disorder, use of psychiatric medication is typically an important component of treatment, and treatment planning frequently requires careful attention to mood symptoms, sleep, stress levels, and longitudinal symptom patterns.

Dr. Shugarman is a board-certified general and forensic psychiatrist and founder of the practice. He provides medication management and psychotherapy for adults. Additionally, he performs forensic psychiatric evaluations and serves as an expert witness.

Dr. Rives is a board-certified psychiatrist. He provides medication management and psychotherapy for persons ages 16 and older. Dr. Rives also has extensive experience evaluating and treating U.S. military veterans.
